Eligibility verification Access member coverage information Search claims Download forms and resources Read the provider manual ## How to register Register for the Provider Portal to make managing member coverage with Sutter Health Plan easy.  - **Step 1:** Go to [shplan.org/providerportal](http://shplan.org/providerportal) and select “Register Here.” - **Step 2:** Carefully review and then accept the “Provider Responsibilities” related to accessing Protected Health Information (PHI). - **Step 3:** Follow the instructions to complete all required sections on the Provider Registration Page. You’ll need your license number, National Provider Identifier (NPI) and Taxpayer Identification Number (TIN). - **Step 4:** We’ll email you to confirm your portal registration request and send a second email once it’s approved. Please allow 2–3 business days for processing. Call our team at [855-315-5800](tel:855-315-5800). **What are the username and password requirements?** Your username: - Can’t be the same as your password - Can’t contain spaces or special characters (such as $ % &) - Must be at least six characters Your password: - Can’t be the same as your username - Must be at least eight characters - Must contain at least one uppercase letter, one lowercase letter, one number (0-9) and one special character (such as $ % &) - As an added layer of protection, we’ll ask you to update your password once a year.   **What types of member coverage information are available on the portal?** The portal includes a comprehensive list of member coverage information, such as:  - Accumulator information (amounts paid toward deductibles and out-of-pocket maximums) - Comprehensive benefit documentation, including EOC and BCM - Member cost share information (including copayments and coinsurance for standard services) - Plan name and type **What claims information can I access through the portal?** The portal allows you to view detailed, line-item claims information. You can search for claims using multiple parameters, including:  - Claim ID  - Date of service range  - Member ID number - Member name **What happens if I forget my username or password?** If you forget your username or password, click the “Forgot Username?” or “Forgot Password?” link, enter your account information and answer the security questions. We’ll email you your username or a temporary password.
