Enrolling Your Clients

View large group plans to find the right options to meet your client's needs.

Contact an Account Executive to discuss plan options or for a quote.

Once ready, confirm your sale via email to your Account Executive. You'll then receive a sold group confirmation intake form to complete and return to start the implementation.

Complete and return the following implementation documents:

• Employer Application (PDF)
• Large Group Subscriber Contract
• Approved Sold Sign-off Sheet
• Sutter Health Plan Enrollment File (CSV) for member enrollment
• ACN Group of California, Inc. Enrollment Agreement (if applicable)
• Approved Sold Sign-off Sheet
• EDI Authorization (if applicable)
  • Initial enrollment is through the Sutter Health Plan Enrollment File (CSV)
  • Standard EDI setup is approximately 4-6 weeks after the group implementation is complete

Email all completed documents to shpserviceteam@sutterhealth.org with the group name and requested effective date in the subject line. You’ll receive an email confirmation. Incomplete submissions may delay processing.

Pay the initial premium online or via check.

Upon approval, you'll receive:

• Group approval letter
• Member roster
• Countersigned contract
• Final benefit plan documents
• Copy of the final Employer Application

Request a quote through HealthConnect or Quotit. We also partner with general agencies AMWINS Connect, CRC Benefits, Word & Brown, Emerson Rogers and Warner Pacific.

Complete and return the documents in the Small Group Submission Checklist (PDF) to shpserviceteam@sutterhealth.org. Be sure to include the group name and desired effective date are included in the subject line. Once submitted, you'll receive a confirmation email. Please note that incomplete submissions could delay processing and affect the effective date.

You'll receive an email containing presale benefit plan documents. The email will also include a Summary of Benefits and Coverage Notice, the group subscriber contract for your client’s signature and a request for any additional information.

Upon approval, you'll receive an email containing:

• Copy of the final Employer Application
• Countersigned contract
• Final benefit plan documents
• Group approval letter
