Member Programs

As a Sutter Health Plan member, you have access to no-cost programs that support your health at every stage — whether you are focused on prevention, managing a condition or building healthier habits. With personalized guidance and early support to address health risks, these programs make it easier to stay engaged in your wellness.

Doula Care

Mahmee provides personalized support throughout pregnancy, delivery and the postpartum period. Mahmee's team includes doulas, registered nurses, mental health coaches and nutritionists, offering a range of services through virtual visits, in-person care, 24/7 support and more. 

 

You can self-enroll by visiting mahmee.com, clicking on ‘Check My Eligibility’ and following the instructions or calling Mahmee at 818-431-1118.

Vida Health

Vida is a virtual health program that supports weight management and helps prevent or manage chronic conditions such as diabetes, high blood pressure and high cholesterol. Through Vida you can connect with accredited health coaches and registered dietitians through video visits and secure in‑app messaging. You can also track food, exercise and sleep habits.

 

You can self-enroll by visiting vida.com/sutterhealthplan or calling Vida at 833-732-2242.

Sutter Care Management

Health Coaching Program 
Members can access the free Health Coaching Program, which offers personal coaching and support for weight management, quitting tobacco and reducing stress. Getting started is simple. You’ll work one-on-one with a trained coach who will help you overcome obstacles, align your actions and set practical goals for a healthier lifestyle.

Complex Case Management Program

If you have chronic conditions or multiple health concerns, you may qualify for our Complex Case Management Program. This program connects you with a team of nurses, social workers, pharmacists and care coordinators who collaborate with your providers to manage your care and explain your medications.
 

To enroll or learn more, call the Sutter Health Care Management program at 855-771-4405 or your assigned medical group (as shown on your member ID card).

Transition of Care

The transition of care service helps you safely go from one care setting to another, such as from a hospital to your home or a skilled nursing facility. From the first day of your hospital stay, discharge planners, case managers and healthcare professionals assess your needs and coordinate your post-discharge care. They also notify your primary care physician (PCP) of your admission and involve them in your care and transition process.

You may opt out of the program when the Sutter Health Care Management team contacts you after discharge.
