
Individual and Family Plan Coverage
With Sutter Health Plan, you have coverage you can count on and access to a network of care you trust. Learn how to enroll in an individual and family plan (IFP).

To enroll or make changes to your coverage, you must experience a qualifying event. Common qualifying events include getting married, having a baby or losing your current healthcare coverage. For a full list of qualifying events, view the enrollment form (PDF).
Follow these steps to enroll during the Special Enrollment Period:
- Step 1: Check if you live in the Sutter Health Plan service area.
- Step 2: View our plans (PDF), rate guide (PDF), benefit plan documents and make your selection.
- Step 3: You can complete your enrollment online or download the enrollment form (PDF).
- Step 4: Submit your signed and completed IFP enrollment form by email, fax or mail. Be sure to include all the required information, as missing details could delay processing. Please don’t include payment with your application.

How to Pay Your First Premium
If you’re applying for coverage as a new policyholder or applying on behalf of one, please make your first month’s premium payment either online or by check. To avoid delays in processing, don’t include your application with your payment.
- Pay online: Pay your first month’s premium through the Sutter Health Plan Online Payment Center.
- Pay with check: Complete the Remittance Slip on page 10 of the enrollment form and make your check payable to Sutter Health Plan. Mail your first month’s premium and completed Remittance Slip to:
