Enrolling Your Employees

View small group plans to find the right options to meet your employees' needs.

Work with an Account Executive or broker to learn more about available plan options or request a quote.

Complete and return the documents in the Small Group Submission Checklist (PDF) to shpserviceteam@sutterhealth.org. Be sure to include your group name and desired effective date in the subject line. Once submitted, you'll receive a confirmation email. Please note that incomplete submissions could delay processing and affect your effective date.

Pay your initial premium online or via check.

Please email a copy of your check or online payment confirmation to shpserviceteam@sutterhealth.org.

You'll receive an email containing presale benefit plan documents. The email will also include a Summary of Benefits and Coverage Notice, the group subscriber contract for your signature, and a request for any additional information.

Upon approval, you'll receive an email containing:

• Copy of the final Employer Application
• Countersigned contract 
• Final benefit plan documents 
• Group approval letter
