How to Register
Registration is easy. To begin, you'll need four things:
- Your member ID number (you can find this on the front of your insurance card).
- Your date of birth.
- Your phone number.
- The last four digits of the subscriber’s Social Security number (SSN). The subscriber is the primary person on the health plan. If that's you, use your number. If you're on someone else's plan, use the last four digits of their number.
