How to Submit Commercial HMO Claims

Please submit Sutter Health Plan HMO members' out-of-area emergency and urgent care claims to the following address, which you'll find on the back of members’ ID cards:

Please be aware that:

  • Providers have 180 calendar days from the date of service to submit claims to Sutter Health Plan.
  • We acknowledge paper claims within 15 business days of receipt, starting from the first business day they arrive.
  • If a claim is paid or denied within these 15 days, the remittance advice serves as the acknowledgment.
